電子消費券｜$5000消費券今派發書面申請表 一文看清申請步驟＋派表地點｜附申請表下載連結

政府將向每名合資格市民派發5,000元電子消費券，7月4日至8月14日期間可透過電子或書面登記，最快8月1日收到第一期2,000元消費券。政府今日（28日）起在官方網站上載書面登記表格以供下載，市民亦可選擇到348個指定地點索取表格，7月5日開始收表。

由6月28日起，市民可於政府「消費券計劃」網站下載書面登記表格（下載連結），或到各區121間郵政局、20間民政事務總署轄下各區民政諮詢中心、11間社署分區福利辦事處，以及196間房委會轄下公共屋邨辦事處和客戶服務中心索取表格，以下為詳細名單：

書面登記表格申請步驟

第一步：市民須在表格填寫香港身份證上的英文姓名、中文姓名（可選擇填寫）、 香港身份證號碼及簽發日期、本地流動電話號碼、用作收取消費券的儲值支付工具帳戶號碼，以及就「在港居住」要求作出聲明。

第二步：7月4日起，市民可將填妥及已簽署的登記表格，連同香港身份證副本釘好及對摺，於辦公時間投入上述郵政局或以下指定銀行分行內的表格收集箱；7月5日開始收表。以下為有關銀行收集箱的詳細地點：

在申請人資格獲核實後，第一期消費券2,000元一般會於9月1日發放到申請人指定的儲值支付工具帳戶；申請人一般會於遞交書面表格後約兩星期收到有關登記結果的通知。
